Oneonta Community Health Center

Eligibility

Currently, there is no residency requirement.

To be eligible:

+ Household incomes may not exceed 200% of the 2011 poverty level established by the Federal Poverty Guidelines.

+ The income of all members of a household will be considered in establishing eligibility. Documentation of income, ie. paystubs, tax returns will be requested at time of visit.

+ The patient must not be eligible for private, county, New York State or Federal insurance programs, such as, HealthyNY, Family Health Plus, Medicaid or Medicare. Patients will be guided to existing insurance programs if the income and employment status indicates possible eligibility.

+ We will not duplicate services; if a patient qualifies for other programs, we will assist in the referral.
